    We hope that you are 100% satisfied with any Daylight product but if you need to return it, please complete the form below.

    We need the proof of purchase if your batch number is older than 2 years. Your lamp has a label on the base or arm with this information.

    Was the fault gradual or sudden? Is it permanent or itermittent? How long did it work for before failing? Have you tried with a new adapter/bulb?

    Please note if you have selected “Replacement” this will be added to your next PO on the bottom (if you are an online customer we will work with you to get a replacement sent straight-away).
